I'm just gonna go with destroying the Caretaker's Array in the very first episode. If they had just gone home - like the PD told them to - then none of her other fuckups would ever have happened.

Going after Chakotays son in Basics.

If losing ones ship is worthy of a court martial, is losing a ship to an enemy not also sufficient cause for one? Even if the ship is retaken, the destructive capabilities of just one Federation ship suggests in my view it should never fall into the hands of the enemy - even if the crew has to fight to the last man.

3) Making Torres Chief Engineer ("Parallax")
4) Releasing the Viidians ("The Phage")
5) Not asking Q to send them home ("Death Wish")
6) Chasing after Chakotay's supposed son ("Basics")
7) Barging into Swarm space ("The Swarn")
8) Trying to grab the Ferengi instead of just heading home ("False Profits")
9) Not asking Q to send them home ("The Q and the Grey")
10) Not kicking Neelix off the ship for lying to her, theft, etc ("Fair Trade")
11) Preventing the destruction of the Borg ("Scorpion")
12) Barging into Bo'mar space ("The Raven")
13) Continuing to try and get through Krenim space after the first attack ("Year of Hell")
14) Charging through a dangerous nebula ("One")

Just about every third minor decision, and half the major ones seemed designed to screw with her crew, random aliens, or just get herself and everyone around her killed for no good reason.

he wasn't helpless he could have used his uberbrain somehow in an A-teamesque way and made a runabout from some vulcan soup and a light fitting....like MacGyver!
