That would involve a) planning ahead, b) having the status quo change as a result of the events of a given episode and c) not being able to pad episodes with stock shots. Over seven years the Voyager production staff proved incapable of doing any of those things. Why do you think the names of Berman and Braga are treated like mud? Another question about the Doctor: does he have rank? Other Starfleet doctors have rank (Crusher even served shifts on the bridge!) like real naval doctors do. Of course, he has the authority to issue doctor's orders either way...
