Why is there only one copy of the Doctor?

That would involve a) planning ahead, b) having the status quo change as a result of the events of a given episode and c) not being able to pad episodes with stock shots. It's because the pair of them (Braga especially) are wedded to the concept of churning out hundreds of episodes based around the same basic plot, without anything resembling long-term character development.

Another question about the Doctor: does he have rank? Other Starfleet doctors have rank (Crusher even served shifts on the bridge!) like real naval doctors do. Of course, he has the authority to issue doctor's orders either way...

He only had rank as the ECH. Other then that he only has the authorty of a Doctor.

Well, I looked it up. DITL>Starfleet Crew>Voyager>EMH>Images

No pips on his collar, and the caption says, in yellow "Voyager's EMH has no offical rank or power, but is nontheless a valued member of the crew.", with reference to the Season 2 episode 'Tattoo'.
