Pleading with Q in "Q Who?" - shame Janeway couldn't do the same and swallow (shut it Tsu) her pride.

I was going to go for that, but Picard did go all Captain Ahab at the end of the movie, so maybe Starfleet had a point. Sending back the Enterprise C through the anomaly.
That was a though choice to make not based on logic and common sense but on a small chance of success and the trust he placed in Guinnan. Most would have dismissed the idea of sending her back and wouldn't be able to grasp the possibly bigger picture.

Since kost has already brought up the best, I'll go for giving Data command of the Sutherland.

Giving Admiral Satie rope until she had an audience to perform in front of, then calmly putting a noose around her neck and dropping her through the trapdoor in the space of about five minutes flat.
Not a "command decision" as such, but "THERE ARE FOUR LIGHTS!" - especially when it's revealed that he had broken, and was actually lying about what he saw.
