Nowhere is it stated that this is so. In the Chase they state that problems came up during development, but the reason given for shelving the Defiant was that "the Borg threat became less urgent", presumably a reference to the defeat of the first cube at Earth. Had they decided to continue with development, it's entirely likely that they would have fixed the problems just as quickly as O'Brien did. Or quicker.

We actually saw very little of the war against the Dominion, though. The great majority of that war happened offscreen. Nor do we know if the parts we did see were remotely representative of what we didn't.

True. There could easily be 200 Defiants out there on the front lines. Then there could be 20.

Considering that what we saw was a mix match of different ship types, you could argue the other fleets could be similar in con position too. Unless Starfleet really does like DS9 to have a bunch of GCS. Of course if the fleet composition was similar to other areas of,conflict, then Starfleet has alot of Galaxy class ships.

Which it probably does, given that it's the single most important strategic location in the entire quadrant.
