I thought it happened long before that, myself. For me it was when Maquis and Starfleet folks were joking and laughing together whilst Janeway commented on how everybody was finally bonding... in like the third episode.

Certainly the moment when Voyager was mortally wounded. Huge potential for drama, really the most intriguing thing about the series, thrown out the window.

Okay, yeah. Maybe it happened a bit before this episode. But this was another mortal blow.
