Nemisis obviously, or just about anytime he invoked the PD when someone was facing death.
In the beginning the Universe was created. This has made a lot of people very angry and been widely regarded as a bad move.

Hence the back story of my fiction.........another guy decides to act, and gets bounced out of the service.
I'm going with giving a juvenile an acting commission, assigning him to a position that would normally be manned by a trained officer, and letting him fly the ship when he's not old enough to drive a car.

Refusing to give aid to the two cultures: one of the Druggers who gave a people a 'cure' for a disease that wasn't needed and got the other people hooked and refusing to help the other people get through their withdrawal symptoms on grounds of the Prime Directive.
Dear god, that was so morally reprehensible, it ranks up there with Archer refusing to give a cure to a civilization because of a non-existent Prime Directive.

I actually can't fault his actions in Nemesis too much.
Well, aside from the end bit where he goes alone to the Scimitar to face Shinzon in a big showdown. That was pretty damn stupid.
I think his worst decision would be any time he let a planet die due to the PD. Yes, he was following orders, but as we all know from a certain trial a few decades ago that's no excuse.

3) Not heading home immediately in "Q Who?"
4) Returning Jeremiah Rosa to the Talarians ("Suddenly Human")
5) Not locking Maxwell in the brig when they first caught up with him ("The Wounded")
6) Not releasing Hugh with the anti-Borg virus ("I, Borg")
7) Hailing the Dyson's Sphere ("Relics")
8) Not deleting Moriarty ("Elementary Dear Data")
