Its a cool idea, and kinda plausable.
What does defeat mean to you?

Nothing it will never come. Death before defeat. I don’t bend or break. I end, if I meet a foe capable of it. Victory is in forcing the opponent to back down. I do not. There is no defeat.

There are a couple other possibilities for the beam not aiming where it should:
The emitter may be adjustable, allowing people to move it around to fire at whatever angle they prefer.
Poor quality control leads to the emitters not being 100%, leading some to send their beams in odd directions.

I file auto-targetting systems in phasers under the "possible but highly unlikely" category.

Maybe the angle of the emitters has been adjusted to compensate for the horribly non-ergonomic designs of the weapons?

IIRC, the whole thing came about because (due to the craptastic design) the actors couldn't aim the phasers in the proper direction. This led to the SFX team having to make the beam come out at all sorts of crazy angles for the weapon to hit its target.

For example Riker aiming for a head shot yet the beam connecting with a body shot.

Of course, you shouldn't be aiming for head shots in combat anyway...
